Running Injuries in St. Louis: How to Treat Them Without Stopping Training

“Just take two weeks off” is not a treatment plan.

If you’re a runner — recreational, competitive, or anywhere in between — you already know that rest alone rarely fixes a running injury. You stop, feel better, go back to running, and the pain returns. The injury hasn’t been addressed; it’s been temporarily quieted.

At St. Louis Sports Clinic, we treat running injuries the way sports medicine teams treat professional athletes: identify the root cause, address it directly, and build a plan that keeps you training through recovery as much as possible.

The Most Common Running Injuries We Treat

IT Band Syndrome (ITBS)
Lateral knee pain in runners is most often IT band syndrome — and it’s almost never caused by a “tight IT band.” The real drivers are typically hip abductor weakness (glute med and TFL), hip drop mechanics during the stance phase, and altered foot strike patterns. We assess the whole chain, not just the knee.

Plantar Fasciitis
Heel pain in runners is among the most common presentations we see. Acute plantar fasciitis responds well to dry needling, soft tissue work, and calf loading programs. For chronic cases — pain lasting more than three months — we add shockwave therapy, which has strong evidence for plantar fasciosis and gets results when other treatments have stalled.

Achilles Tendinopathy
Mid-portion and insertional Achilles pain are both common in distance runners, particularly those ramping up mileage or transitioning footwear. Progressive loading through a structured tendon rehabilitation program is the cornerstone of care — we combine it with dry needling, soft tissue work, and shockwave when indicated.

Shin Splints (Medial Tibial Stress Syndrome)
MTSS is an overuse injury to the tibial periosteum, usually caused by rapid increases in volume, inadequate recovery, or abnormal tibial loading mechanics. We differentiate MTSS from stress fractures (serious), address the muscular and biomechanical contributors, and modify training load appropriately.

Stress Fractures
Tibial and metatarsal stress fractures require accurate diagnosis — something our imaging specialist, Dr. Jessica Billham (DC, DACBR, RMSK), can support with diagnostic ultrasound and imaging interpretation. Return-to-running timelines are sport-specific and carefully managed.

Hip Flexor and Psoas Injuries
Hip flexor strains and psoas tendinopathy are common in speed-focused runners and those increasing tempo or hill work. We address these with targeted soft tissue work, dry needling, and progressive hip rehabilitation.

Patellofemoral Pain (Runner’s Knee)
Anterior knee pain in runners is often patellofemoral — driven by quad weakness, hip abductor deficits, and altered mechanics at foot strike. Load management combined with targeted strengthening resolves most cases without any passive treatment at all.

How We Actually Assess Running Injuries

Most running injuries are not caused by the thing that hurts. IT band pain comes from the hip. Plantar fasciitis comes from the calf and hip. Patellofemoral pain comes from the quad and hip. Achilles problems come from the soleus and training load.

If your care provider is only looking at your knee because your knee hurts, they’re missing most of the picture.

At St. Louis Sports Clinic, we use the Selective Functional Movement Assessment (SFMA) and the GRIP Approach to assess your full movement chain — starting with how your hips, ankles, and thoracic spine move, and working from there to identify the breakdowns that are driving your injury. We watch how you move. We watch how you load. If it’s relevant, we watch how you run.

Then we build a treatment plan that matches what we actually find.

Treatment Options for Running Injuries at St. Louis Sports Clinic

Dry Needling: All nine of our providers are certified in dry needling. For running injuries, we commonly target the glute med, TFL, calf complex, tibialis posterior, hip flexors, and plantar intrinsics — the muscles that are overloaded, inhibited, or carrying trigger points that are contributing to pain.

Shockwave Therapy: For chronic Achilles tendinopathy, plantar fasciitis, and shin splints with periosteal involvement, shockwave accelerates tissue healing in ways that passive rest cannot.

Soft Tissue Therapy & ART: We break down adhesions and scar tissue in the calf, IT band fascial system, hip flexors, and plantar fascia. Active Release Technique (ART), instrument-assisted soft tissue techniques (IASTM), and cupping all play a role depending on what we find.

Progressive Loading & Rehabilitation: We don’t just treat and discharge. We build structured progressive loading programs for the tendon, muscle, or joint involved — because the research is clear that progressive loading is more effective than rest for most running injuries.

Chiropractic Manipulation: Restricted ankle dorsiflexion, limited hip mobility, and locked thoracic rotation all contribute to running injuries. We address joint mobility directly with targeted manipulation.
